Social Work
Social work promotes positive change, brings people together, and supports people to overcome barriers preventing them from meeting their full potential.
Social work aims to empower individuals, families, groups, communities, and society more broadly.
A social worker is often the first point of contact in times of hardship: relationships, finances, family, housing, drug and alcohol problems, complex health system/disability issues, among other challenging situations.
Our social work services include:
- Finding solutions to complex systemic problems related to your disability
- Advocacy support
- Liaising with services to meet your specific disability needs
- Enabling community participation
- Building your confidence and capacity to achieve your goals
- Identifying and using your strengths
- Developing your resilience
- Housing and accommodation such as Supported Independent Living (SIL), Independent Living Options (ILO), and Specialist Disability Accommodation (SDA)
- Advanced care planning
- Guardianship and Administration orders
- Hospital discharge planning
- NDIS plan review reports
- Navigating government systems such as My Aged Care, NDIS, Housing SA, and Centrelink
If you have Capacity Building Supports (Improved Daily Living) funded in your NDIS plan and would like to discuss whether our services might be right for you, please contact us.
